While most pet owners simply keep the health records for their pets in a file, other information on the form can come in handy in case of a natural disaster, your pet getting lost, or needing a pet sitter. In the case of a natural disaster where your family and pets need to be evacuated, evacuation personnel may require that your pet be kenneled. If your pet gets lost or is stolen, both the written description and your pet's picture could help with recovering your pet. If you need to engage a pet sitter, the instructions, as well as which veterinary clinic to use in case of an emergency, are all in one place.

Having all the necessary information such as a photograph, description (get the kids to help), along with your pet's feeding and prescription schedule, and his/her doctor's address and contact information will provide you with peace of mind.
